Les réformes hospitalières de 1996 et le Programme de Médicalisation du Système d'Information (PMSI) réduisent la sélection adverse et le risque moral entre le Ministère de la santé (le principal) et les établissements de santé (les agents). Le recours au concept de " trajectoire patient ", c'est-à-dire à l'étude du déplacement du patient dans le temps au sein de l'espace de soins pour diagnostiquer, traiter et surveiller une pathologie donnée, permet encore d'atténuer l'asymétrie d'information, en particulier la difficulté pour les agences régionales de l'hospitalisation (les superviseurs) de qualifier le résultat des agents. La trajectoire patient apparaît, en l'absence de collusion, comme un instrument performant de planification pour le principal, de contrôle pour les superviseurs et de négociation pour les agents. Outre ses effets positifs sur la relation hiérarchique d'agence, le concept de " trajectoire patient " converge aux cotés de la filière et du réseau de soins vers la notion de prise en charge globale du patient. L'application de ce concept à la cancérologie pédiatrique permet d'illustrer la construction théorique et de comprendre l'organisation de la prise en charge des cancers de l'enfant en Rhône-Alpes. Finalement, l'évolution vers la notion de " trajectoire optimale du patient ", se traduit par une convergence vers une relation d'agence hiérarchique à superviseur unique (une région) et agent unique (le réseau de soins). |

Modes for financing pubic hospitals and optimal patient's trajectory in paediatric oncology.
The hospitals act modified in 1996 and the French hospital medical information system (PMSI) decrease the adverse selection and the moral hazard between the health Ministry (the principal) and hospitals (the agents). The introduction of the concept of patient's trajectory i.e. the analyses of the pathway of the patient over time and within the health system reduces further the asymmetric information, particularly for regional hospital agencies (the supervisors) for evaluating the result of the agents. In the absence of any collusion, the patient's trajectory appears to be a successful planning tool for the principal, a control tool for the supervisors and a bargaining tool for the agents. In addition to its effect on the hierarchical agency relationship the concept of the patient's trajectory converges with the health care network towards the global care of the patient. This concept has been applied to paediatric oncology to illustrate the theoretical representation and to understand the organisation of the care of patients in the Rhône-Alpes region. Finally the evolution towards the concept of the " optimal patient's trajectory " seems to lead to a hierarchical agency relation with one supervisor (the region) and one agent (the health network). |
